AI Phone Assistant: What It Can Do, What It Costs, and Who It’s Worth It For

TL;DR

An AI phone assistant answers calls around the clock, handles frequently asked questions, books appointments and forwards complex requests – with a natural-sounding voice and a live connection to your calendar and CRM. Cost: €29 to €500 a month plus €0.08 to €0.50 per call minute (German market), with flat rates from around €99. It pays off for clinics, trades businesses, law firms and service providers that miss calls every day. Mandatory: GDPR-compliant hosting, a data processing agreement (DPA), and, since August 2026, a clear notice that an AI is speaking.

What is an AI phone assistant?

An AI phone assistant is software that automatically answers incoming calls and holds a genuine conversation with the caller – in natural spoken language, without a keypad menu. It recognises the caller’s request, answers common questions, captures details, books appointments straight into the calendar, and forwards anything it cannot resolve to the right person. Under the hood it combines speech recognition, a large language model for understanding, and a synthetic voice for the reply – all in real time, with response times under a second.

The difference from a classic phone system with recorded announcements and a hold queue is fundamental. The caller simply says what they want: “I need an appointment next week in the afternoon” or “My heating isn’t working, is anyone available today?” The assistant understands the sentence, asks for any missing information, and either resolves the request or hands it over with full context. That is why, in 2026, AI phone assistants are among the fastest-growing AI agents among SMEs.

How does an AI phone assistant work?

Behind every call, a chain of steps runs in a fraction of a second:

  • Speech recognition: what is spoken is converted into text – including accents, background noise and interruptions.
  • Understanding and decision-making: a language model identifies the request, matches it against the company’s stored rules and information (opening hours, services, prices, available slots) and decides what to do.
  • Action: the assistant accesses connected systems – calendar, CRM, ticketing system, practice management software – and carries out the action: booking an appointment, logging a callback, creating a note.
  • Reply: a synthetic voice speaks the answer. Modern voices are barely distinguishable from human ones, including natural pauses and intonation.
  • Follow-up: after the call, the assistant creates a summary, sends it by email or into the CRM, and triggers follow-on processes where needed – for example an appointment confirmation by text message.

Setting one up requires no technical background: with most providers you enter your company information, typical questions and answers, and forwarding rules through a web interface. The phone number is connected via call forwarding – either permanently, only outside opening hours, or when nobody picks up after a set number of rings.

How much does an AI phone assistant cost?

German-market pricing follows three patterns. Rates range from €29 a month for entry-level plans up to several hundred euros for businesses with high call volumes; fully managed enterprise packages sit above that.

ModelPrice range (German market, 2026)Suited to
Base fee + per-minute rate€29–500 per month plus €0.08–0.50 per call minute; included minutes vary by planFluctuating call volume, getting started
Flat ratefrom around €99 per month, sometimes with a setup fee (e.g. €199)Predictable costs at 50–200 calls/month
Per conversationBilled per call rather than per minuteShort, standardised calls, e.g. appointment booking
Industry solutionse.g. medical practices: from around €90 net/month per location including 500 minutes, then around €0.18/minuteClinics and law firms with specialist requirements

Worked example: a trades business handling 150 calls a month at three minutes each reaches 450 call minutes. On a per-minute plan with a €69 base fee including 750 minutes, that comes to €69 – less than an hour of office staff time. But the real calculation is a different one: what does a missed call cost? If just one call in ten turns into a job worth €500, and 30% of calls previously went unanswered, that business is losing more than €2,000 worth of jobs every month.

The prices quoted are market benchmarks (German market) based on public provider information (including Placetel, Zeeg and Vokaro) and change continuously. Hidden costs come from setup fees, additional phone numbers, integrations and minutes beyond the included volume – ask about the effective per-minute rate at your actual call volume.

Which AI phone assistants are available for businesses?

The German market is broad in 2026, and providers fall into three groups:

  • Specialised German providers: fonio, VITAS, Vokaro, SpeakKI, smao and others – built for the German language, GDPR and SMEs, usually with EU hosting and fast setup.
  • Telephony providers with an AI module: Placetel, IONOS, Tenios – a good fit if your phone system already runs on one of these; integration is then seamless.
  • Industry-specific solutions: MediVoice or Aaron AI for medical practices, and specialised assistants for tax firms, car dealerships or estate agents – with pre-built dialogues and integration into industry-specific software.

Which AI phone assistant is best comes down to four questions: does it understand your callers (accents, technical terms, background noise)? Can it connect to your systems? Where is the data processed? And can you test it beforehand? Almost every provider offers a free trial or a demo number – use it with real scenarios from your day-to-day business, not sample questions.

Which industries benefit most from an AI phone assistant?

Anywhere the phone is the most important channel – and nobody has time to answer it:

  • Medical and therapy practices: appointment booking, prescription requests, opening hours – otherwise practice staff spend hours on the phone while patients wait in the waiting room.
  • Trades businesses: fitters are on site, the office is only staffed part of the day. Every unanswered call goes to the next business.
  • Law firms and tax advisers: qualifying initial enquiries, capturing client requests, coordinating callbacks.
  • Car dealerships and garages: workshop appointments, status enquiries, spare-parts orders.
  • Real estate: viewing requests around the clock – prospects call in the evening and at weekends.
  • Service providers and agencies: reachability outside office hours, pre-qualifying enquiries.
  • Hospitality: reservations and menu questions while the team is on the floor.

It is less worthwhile where calls almost always require complex advice, or where call volume is so low that voicemail is sufficient. And: an AI phone assistant does not replace staff, it filters. The AI handles the standard cases, while cases needing advice land pre-qualified with the team – the same principle we describe in our article on AI chatbots in customer service.

Does the AI sound natural – and is it GDPR-compliant?

Voice quality is no longer a problem in 2026: modern speech synthesis sounds natural, responds without noticeable delay, and handles interruptions well. Callers often only realise they are talking to an AI when they are told. That notice is exactly what is required by law: since 2 August 2026, Article 50 of the EU AI Act requires people to be informed when they interact with an AI system – full details in our guide to the AI labelling obligations. Reputable providers build the notice into the greeting by default.

For data protection, three requirements apply: a data processing agreement (DPA) with the provider, hosting and processing within the EU, and information for callers about the processing (a privacy notice, for example referenced in the greeting). Call recordings require consent; transcripts and summaries use less data and are the leaner option. Medical practices also have professional confidentiality obligations to consider – here, only providers experienced in healthcare should be used.

Your company is responsible for what the assistant says. Since the Higher Regional Court of Hamm ruling, statements made by AI systems count as the operator’s own statements – false pricing information or promises about services made over the phone are not an operational slip, they are a competition-law infringement. More on this in our article on AI chatbot liability.

How to introduce an AI phone assistant

  • 1. Analyse your calls: for two weeks, log who calls, why, and when. This gives you the top ten requests the assistant needs to be able to handle.
  • 2. Define the scope: what may the AI resolve on its own (appointments, information), what should it only capture, and what must go straight to a human? Emergencies always need a route to a person.
  • 3. Test providers: have two or three providers handle your real scenarios – including accents, background noise and unclear requests.
  • 4. Roll out in stages: start by covering only out-of-hours periods or busy lines. Review call logs daily and refine the answers.
  • 5. Measure results: number of calls answered, requests resolved, calls forwarded, appointments booked. After four weeks, decide whether the assistant should take over during core hours too.

Our verdict

In 2026, an AI phone assistant is one of the few AI applications whose benefit pays for itself within weeks: every answered call is an enquiry that would otherwise have been lost, and every automatically booked appointment is one fewer interruption. What matters is a realistic scope, a provider with EU hosting and clean labelling – and the nerve to start by letting the AI work only where nobody is picking up today.

FAQ

Frequently asked questions

How much does an AI phone assistant cost?

Between €29 and €500 a month base fee (German market) plus €0.08 to €0.50 per call minute, depending on the included volume. Flat rates start at around €99 a month, and industry solutions for medical practices from around €90 net per location. For a business with 150 calls a month, costs typically stay under €100.

What is an AI phone assistant?

Software that automatically answers calls and holds conversations in natural spoken language. It answers common questions, books appointments, captures enquiries and forwards complex cases to staff – around the clock, and connected to a calendar, CRM or practice management software.

Which AI phone assistant is the best?

There is no single best provider across the board. What matters is how well it understands your callers, how it integrates with your systems, EU hosting with a data processing agreement, and a free trial option. German specialists such as fonio, VITAS or Vokaro, telephony providers such as Placetel, and industry solutions such as MediVoice each cover different needs.

Can an AI phone assistant book appointments and forward calls?

Yes. With a calendar connection it checks free slots, books, confirms and reschedules appointments. It forwards calls according to rules – emergencies immediately, advice requests to the responsible person – and hands over a summary of the conversation so far.

Is an AI phone assistant GDPR-compliant?

Yes, if the provider offers a data processing agreement, processes data within the EU, and callers are informed about the processing. Recordings require consent. In addition, the EU AI Act has required, since August 2026, that callers be told they are speaking with an AI.

Does an AI phone assistant replace staff?

No, it filters. The AI handles routine matters such as opening hours, appointment booking or status enquiries, while anything needing advice lands pre-qualified with the team. The result is fewer interruptions and no missed calls – not fewer staff.
